Surge of Shigellosis in North Dakota Triggers Investigation

A surge in Shigella infections in Rolette County, North Dakota, has prompted investigations. To date, 28 people have confirmed Shigella infections and 10 people have been hospitalized. Attorney Fred Pritzker comments on Shigella infections and investigations.

An unusually high number of Shigella infections in Rolette County, North Dakota, has triggered a government investigation. According to Julie Goplin, foodborne surveillance epidemiologist with the North Dakota Department of Health, there have been 28 confirmed cases of shigellosis in Rolette County. Ten people have been hospitalized.

As with most cases of shigellosis, finding the source is proving difficult. The State Department of Health is following all possible leads but has not been able to confirm or eliminate any of them. Interviews of victims and their families have not pointed to any single potential cause. Because Shigella can be transmitted from person to person, it is possible a food product sickened one or a few people and the others contracted Shigella poisoning (shigellosis) from person-to-person contact. Experts are conducting DNA fingerprint testing to determine if one strain of Shigella is responsible for the illnesses in Rolette County.

Most people who are infected with Shigella develop diarrhea, fever, and stomach cramps starting a day or two after they are exposed to the bacterium. The diarrhea is often bloody. Shigellosis usually resolves in five to seven days. Some persons who are infected may have no symptoms at all but may still pass the Shigella to others.

In some persons, especially young children and the elderly, the diarrhea associated with shigellosis can be so severe that the patient needs to be hospitalized. In children under 2, severe shigellosis with high fever may cause seizures. One type of Shigella can cause Reiter's syndrome, which can last for months or years and can lead to chronic arthritis.

U. S. Employee Engagement Deteriorates as Financial Crisis Worsens

Modern Survey national study indicates employee engagement levels are on the decline as economic woes hit home and at the office.

In the wake of recent economic turmoil, employee engagement figures to be more critical to business health than ever, yet new scientific research shows employee engagement on the decline. A recent national study by Modern Survey (http://www. modernsurvey. com) has shown employee engagement levels dropping in every category measured, including a 7% decline in the number of employees that say they "take pride in their company."

A scientific study of national worker opinions by Modern Survey confirms that employee engagement as a whole has clearly declined over the last year. An astonishing 21% of U. S. workers were actively disengaged as of this past August when the study was conducted, a difference of four percentage points from August 2007.

Included in this national study were five questions that measure the extent to which employees take pride in their company, believe they have a promising future at their company, recommend their company as a great place to work, go "above and beyond" their normal job duties to help their company succeed, and intend to stay with their company.

These questions were first posed to a nationally representative sample of the U. S. workforce in August of 2007, then again in August of 2008. The survey was conducted via the internet, and was completed by 1000 U. S. adults (18+ years old) who matched census data in terms of age, gender and region.

All five components of the Engagement Index show erosion in favorability, and organizational pride shows the largest (and most statistically significant) decline over the last year, dropping from 78% favorable in 2007 to 71% in 2008.

Now only about half of respondents said they are willing to put in extra effort to help their company succeed and only about half say they intend to stay with their company for a long time, both figures reflect a five percentage point drop from 2007.

"You can't open a newspaper or surf the internet without being bombarded by headlines and stories about the country's ever-deepening economic troubles. Month after month, we've all been learning about the subprime mortgage mess, plummeting property values, foreclosures, layoffs, and the collapse of some of our largest financial institutions. People are spending a lot more time worrying about how to make ends meet than they did just a year or two ago," said Bruce Campbell, a Senior Consultant at Modern Survey.

The statistics from this study reflect the current mood of the U. S. workforce as a whole, but what about the engagement levels of individual workers? Modern Survey uses a proprietary method for categorizing individual workers into four distinct levels of engagement, based on the average rating each worker gives across five Engagement Index questions. Fully engaged employees answer at least four of the five questions with the most positive response possible. Moderately Engaged employees give mostly positive answers, while Under Engaged employees give mostly neutral to mildly positive answers. Disengaged employees give mostly negative answers.

Modern Survey's study determined the percentage of U. S. workers at the Moderately Engaged level has fallen from 30% in 2007 to 27% in 2008. On the flipside, the number of Disengaged employees has risen from 17% in 2007 to 21% in 2008.

According to Modern Survey President Don MacPherson, "These findings should serve as a wake-up call to leaders and managers in any type of organization. As economic conditions worsen, you'll be counting more and more on your employees to put forth their best efforts and to pull your organization through. These results suggest that unless you pay special attention to the engagement of your workforce, and to maintaining the type of work environment in which employee engagement flourishes, fewer and fewer of your employees will be willing to "give their all" to help your company succeed."

"It's important to note that our second survey was conducted after gas prices had peaked in July, and after the demise of Bear Stearns, but before the troubles at Merrill Lynch, Lehman Brothers, Fannie Mae and Freddie Mac, or AIG became public knowledge," Campbell clarified. "We know what the basic requirements for having an engaged workforce are, and three of the key levers are giving people opportunities to grow and develop in their careers, providing lots of recognition and sincere appreciation for your people and their accomplishments, and doing whatever you can to make them feel like they are part of a winning team. Given what's happened since August to further fuel workers' anxieties about the economy and about their own futures, smart leaders will be paying special attention to these basic needs of their workforce over the coming months."

Topping the list is the sales figures for opiate panels. Opiate drugs are often prescribed to relieve pain. Examples include Vicodin, OxyContin and Percodan.

Closely following sales figures for opiate panels are those for benzodiazepines and buprenorphine. Benzodiazepines like Valium, Ativan and Halcion are used to treat sleep disorders and anxiety. On the other hand, drugs containing buprenorphine are actually used for treating opioid addiction withdrawal.

These trends coincidentally parallel the 2008 results of the annual survey made by the National Institute on Drug Abuse (NIDA) through its Monitoring the Future (MFT) program. According to the NIDA fact sheet, 15.4% of teenagers in the United States reported to have gone through prescription drug abuse (http://www. testcountry. com/products. html? product=1834) in 2008.

The survey further revealed that the substances being taken non-medically by these teenagers continue to be tranquilizers, barbiturates, amphetamines, opiates and sedatives. Other drugs being abused are those readily available at drugstore counters, such as cough medicine.

But while the numbers describing prescription drug abuse among teenagers is considered as alarming, this percentage is still thought of as low when compared to that of teenagers struggling with the use of marijuana and other cannabis-derived drugs. Marijuana remains to be the most commonly abused substance among teenagers in the United States. Of the teenagers surveyed by the MFT in 2008, 26.9% have reported using it.

NIDA has reported that stress is one of the contributing factors behind prescription drug abuse among teenagers. The need to fit in and to perform well, coupled by peer pressure and lack of guidance from parents or guardians, can cause teenagers to cope with the use of prescription drugs.

Lack of information has also led teenagers to believe that prescription drugs are safer than illegal substances like cocaine or heroin because they are prescribed by licensed doctors and are bought from a pharmacy.

Prescription drugs are also the ones most readily available to teenagers. They can get their dosage from the medicine used by their parents and siblings. They can also persuade friends with conditions such as ADHD to give them some of their medicine.

Despite the concerns raised by prescription drug abuse among teenagers, NIDA has nonetheless stated that there is a steady decline in substance abuse in that age group. Past-year use of illegal drugs aside from marijuana is down from 13.1% in 2007 to 11.3% in 2008. The same goes for the use of crystal meth, from 1.6% to 1.1%.

In NIDA’s prescription drug abuse research, parental supervision is named as one of the factors deemed crucial towards the prevention of non-medicated use of prescription drugs among teenagers. Norphar. com compared Medigap rates used for Central Pennsylvania with the prescription services from Canadian pharmacies. It shows (using an average from three major companies) that purchasing prescription drugs from Canada is significantly cheaper than using Medigap coverage.

Norphar. com compared the difference between Medigap prescription coverage and using a Canadian Interent pharmacy to fulfill the prescription needs of someone on Medicare.

In this example, we decided to find out how benefitial using a Medigap plan that included prescription drug coverage was compared to buying prescription drugs from a Canadian pharmacy.

We used information from an average cost state and 70 year age group.

We took the difference between the prices of Medigap plans E and H, since they most closely resemble each other among the different Medigap plans aside from drug coverage.

We used three different well known insurance companies to compare the plans.

We took the price difference between Plans E and H (70 year old male) for each of the three companies and averaged those differences. The average difference between plan E and plan H was $978 per year.

This represents the amount payed for prescription coverage in plan H. The Prescription coverage in plan H consists of a 50% copay up to $1,250 worth of coverage with a $2,500 purchase limit.

So it costs $978 a year to get 50% off of $2,500 worth of prescription drugs under Medigap plan H.

Any purchase exceeding $2,500 for the year costs full price.

Canadian Pharmacies (www. norphar. com) often offer prices 50% or more off prescription medication with no limit on purchases and require no premium.

So this leads to the conclusion that purchasing one of the Medigap Plans A – G (plans with no prescription coverage) and using a Canadian pharmacy to fill prescriptions, will save the consumer $978 per year (the premium payed for prescription coverage under plan H) and put it in his pocket.

This information has been presented simply as a comparison tool and the numbers will vary by many different factors including age, geographic location, type of drugs purchased, physical health etc. In order to get hard numbers for your own use you need to do the homework or hire a professional to help you.
