The City of Tracy has joined a number of cities in the State of California in adopting an Emergency Medical Services Cost Recovery Fee Program. Effective at a date to be announced, a $300 fee for Tracy residents will be charged for services rendered for responses that meet the criteria for advanced life support and/or basic life support as defined by the California Health and Safety Code (Sections 1797.52 and 1797.60). The City Of Tracy does not charge for calls made to the 911 system and there is no plan to do so. The new cost recovery fee and related household and business membership program are efforts to help offset rising costs for public services alongside reduced general funds.

The City of Tracy has joined a number of cities in the State of California in adopting an Emergency Medical Services Cost Recovery Fee Program. Effective at a date to be announced, a $300 fee for Tracy residents will be charged for services rendered for responses that meet the criteria for advanced life support and/or basic life support as defined by the California Health and Safety Code (Sections 1797.52 and 1797.60).

Begun in southern California, the cost recovery program helps pay for the optional emergency medical services fire departments provide their communities. Members of the Tracy community will pay $300 for emergency services meeting the criteria established. People from outside Tracy can be billed $400. In either case, these fees can be submitted by individuals to their insurance carriers for partial or whole reimbursement.

In an effort to offset any sudden payment, citizens can opt for a subscription service of $4 a month or $48 annually ($36 for low income households). This allows individuals a chance to avoid ever having to pay the $300 fee. There are additional membership options for businesses.

In fiscal year 2008-2009, the City of Tracy Fire Department responded to 2,500 medical emergencies. Fire Division Chief David Bramell states about the service cost recovery program that “EMS fees are based on the philosophy of funding “public good” services (those benefiting everyone) with broad general taxes and “private good” services (those benefiting specific users) with appropriate user fees. General tax revenue continues to provide the vast majority of funding for fire stations, equipment and staffing while these user fees will supplement specific equipment and training required for those individuals who provide emergency medical services to residents and non-residents.”

In addition, Chief Bramell points out that the Fire Chief can waive the fee in exceptional or unusual circumstances. City Manager Leon Churchill emphasized that the City will make the final determination on collections, not an outside third party and encourages its citizens to join the EMS Membership Program to avoid any sudden fees. The City will publicize the membership program option and in addition, will work with individuals and their insurance carriers when possible to collect the use fees. There will be an appeals process for any program complaints.

City Manager Churchill describes “During these times of increasing public service costs and decreasing general funds to pay for these services, governments must be smart in their implementation of plans and strategies that continue to offer high quality services with fair transfer of associated costs to those who benefit from those services. The City of Tracy offers its EMS Membership Program as a solution for its community that is affordable and straightforward for city staff to manage.”

The EMS Membership Program is a voluntary program open to residents of the City of Tracy, and any business located in and possessing a business license issued by the City of Tracy. Eligibility may be established through current utility bills, rent receipts, or other forms of identification which clearly reflect residence or business location within the City. Participants in good standing of the EMS Membership Program that do not have insurance that would otherwise cover the costs will not be charged the EMS Fee if service is rendered.

Doctors treat millions of children with Ritalin every year to improve their ability to focus on tasks, but SCSF scientists now report that Ritalin also directly enhances the speed of learning.

In animal research, the scientists showed for the first time that Ritalin boosts both of these cognitive abilities by increasing the activity of the neurotransmitter dopamine deep inside the brain. Neurotransmitters are the chemical messengers neurons use to communicate with each other. They release the molecule, which then docks onto receptors of other neurons. The research demonstrated that one type of dopamine receptor aids the ability to focus, and another type improves the learning itself.

The scientists also established that Ritalin produces these effects by enhancing brain plasticity – strengthening communication between neurons where they meet at the synapse. Research in this field has accelerated as scientists have recognized that our brains can continue to form new connections – remain plastic – throughout life.

“Since we now know that Ritalin improves behavior through two specific types of neurotransmitter receptors, the finding could help in the development of better targeted drugs, with fewer side effects, to increase focus and learning,” said Antonello Bonci, MD, principal investigator at the Ernest Gallo Clinic and Research Center and professor of neurology at UCSF. The Gallo Center is affiliated with the UCSF Department of Neurology.

Bonci is co-senior author of the paper, which will be published online in “Nature Neuroscience” on Sunday, March 7, 2010.

Bonci and his colleagues showed that Ritalin’s therapeutic action takes place in a brain region called the amygdala, an almond-shaped cluster of neurons known to be critical for learning and emotional memory.

"We found that a dopamine receptor, known as the D2 receptor, controls the ability to stay focused on a task – the well-known benefit of Ritalin,” said Patricia Janak, PhD, co-senior author on the paper. “But we also discovered that another dopamine receptor, D1, underlies learning efficiency.”

Janak is a principal investigator at the Gallo Center and a UCSF associate professor of neurology. Lead author of the paper is Kay M. Tye, PhD, a postdoctoral scientist at the Gallo Center when the research was carried out.

The research assessed the ability of rats to learn that they could get a sugar water reward when they received a signal – a flash of light and a sound. The scientists compared the behavior of animals receiving Ritalin with those that did not receive it, and found those receiving Ritalin learned much better.

However, they also found that if they blocked the dopamine D1 receptors with drugs, Ritalin was unable to enhance learning. And if they blocked D2 receptors, Ritalin failed to improve focus. The experiments established the distinct role of each of the dopamine receptors in enabling Ritalin to enhance cognitive performance.

In addition, animals that performed better after Ritalin treatment showed enhanced synaptic plasticity in the amygdala. Enhanced plasticity is essentially increased efficiency of neural transmission. The researchers confirmed this by measuring electrical activity in neurons in the amygdala after Ritalin treatment.

The research confirmed that learning and focus were enhanced when Ritalin was administered to animals in doses comparable to those used therapeutically in children.

“Although Ritalin is so frequently prescribed, it induces many brain changes, making it difficult to identify which of those changes improve learning.” said Kay Tye. “By identifying the brain mechanisms underlying Ritalin’s behavioral enhancements, we can better understand the action of Ritalin as well as the properties governing brain plasticity.”

Other co-authors on the paper and collaborators in the research were Jackson Cone and Lynne Tye, who were undergraduate assistants at the time of the study, and Evelien Hekkelman, a medical student working with Kay Tye at the Gallo Center.

Jump starting spring fitness, Fit4D™ (Fitness4Diabetics), and the Juvenile Diabetes Research Foundation (JDRF) announce collaborative efforts to provide virtual diabetes educational information to the participants of the JDRF Ride to Cure Diabetes, 2009. The Fit4D Coaches, Certified Diabetes Educators, will present three virtual online webinars to the 1500 Ride participants and their cycling coaches to prepare for the ride.

1 – Determine if the property is eligible:

Anchored or unanchored single-story retail centers are eligible for Retail Property Loans:

Anchored – Allowable anchors include supermarket-drug stores, discount department stores, dry goods, retail and home improvement stores. Financially healthy national, regional or local chains are acceptable. Anchor tenant leases should have at least five years remaining on their leases, as of the date of closing. Anchors should exhibit strong sales histories. Unanchored – Unanchored centers should have a complimentary tenant mix. Stores in unanchored centers must have strong, stable sales histories. Not more than 25% of the leases should expire in any single year. Credit tenants with base lease terms exceeding five years beyond the final term loan typically will receive most favorable underwriting.

The property must be strong sales generator with a good reputation in the market. Two-story centers can be considered, but typically will underwrite the second story with income limitations. Special use properties and smaller buildings with limited appeal to alternative tenants are excluded. Free-standing stores are considered on a case-by-case basis. Current minimum economic occupancy of 85% is required. And credit tenants with greater than five years remaining on the lease term are preferred.

2 – Determine eligible property locations:

Nationwide - require direct access to major roadways and high visibility. Infill locations in developed neighborhoods are preferred. Unanchored centers should be located in high traffic areas. Where direct competition exists, the property is required to exhibit a stronger market appeal than the competition or a history of retaining its tenancy, sales volume and competitiveness.

3 – Identify the Type of loan desired:

Acquisition, Development, Construction, or Permanent Commercial Real Estate Financing is available.

4 – Identify Details of the Loan:

Loan size – a typical retail property loan ranges from $3 Million - $100 Million, larger portfolio transactions will also be considered Debt Service Coverage – Generally, 1.20 to 1.30x, depending on the quality of the location of the market, the property and the existence and quality of anchor tenants. Loan-to-Value Ratio – Up to 80% maximum LTV in first position for permanent financing. Higher leveraged requirements should inquire about mezz debt options. Long Term – 7, 10, 15, 20 and 25 years are available for permanent financing. AD&C loans from 12 – 36 months. Amortization – 30 years or less, depending on major lease terms and expiration, and property age. *Interest Rates – Pricing is based upon the quality of the real estate and the credit strength of the deal.

EyeCare America's AMD EyeCare Program is a year-round program that promotes annual eye exams for people 65 and older, raises awareness of AMD, provides the latest information about treatment options and low vision services, offers free AMD educational materials, and facilitates access to care at no out-of-pocket cost for those who qualify. The eye exams are provided by more than 7,000 dedicated volunteer ophthalmologists across the U. S. People may call the toll-free helpline at 1-866-324-EYES (3937), anytime, for themselves, family members and/or friends to see if they qualify to be referred to one of our volunteer ophthalmologists, or to request free AMD educational material.

Callers who have not seen an ophthalmologist in three or more years may be eligible to receive a comprehensive medical eye exam and, in most cases, up to one year of care at no out-of-pocket cost for any disease diagnosed during the initial exam.

The AMD EyeCare Program is designed for people at increased risk for AMD, but who have NOT been diagnosed and who:
 Are age 65 and older  Are US citizens or legal residents  Have not seen an ophthalmologist in three or more years  Do not belong to an HMO or the VA Eyeglasses, medicines, hospital services and fees of other medical professionals are not included.
