Ventricular assist devices (VADs), blood pumps used in heart failure situations, now have the potential for use in additional patient groups. Scientists and clinicians gathered at the 11th Congress of the International Society of Rotary Blood Pumps in Germany last year to discuss past lessons learned and future directions for this technology.

Increasing experience, better understanding, and advances in VADs now suggest that its smaller size offers an advantage in permitting its use in women and smaller patients, including children with heart failure. These miniature devices, such as the MicroMed DeBakey Child VAD, have the potential to bring about similar results in children as they do currently in adults, sustaining heart function, until the heart can recover or until a heart transplant can be done. According to WebMD Health, as many as 50,000 people each year could benefit from a heart transplant, but only 2,000 to 2,500 hearts become available. As a result, tens of thousands of patients each year die while waiting for a heart transplant.

The development of VADs was derived from the first successful clinical use of the heart-lung machine. Researchers actively set out to discover other ways to support a failing heart to allow more time for recovery. The article relays how the efficacy and safety of VADs were demonstrated in numerous trials along the way. After 50 years of use, VADs are still considered to be in their infancy, but today, the technology has contributed to prolonging support for patients, and has helped to maintain healthy, functioning hearts.

“[This is] of significant value in improving the treatment of intractable heart failure,” states Dr. Michael DeBakey, author of the editorial in the journal, Artificial Organs. Researchers are hopeful for further advancement in the technology but stress the need for commitment from leaders in the field in addition to “adequate reimbursement.”

The author of a hard-hitting book on the sign industry says that modern procurement trends are sending the signal that some large companies ‘don’t care about health and safety’.

Steve Martin, Managing Director of sign installation and maintenance company Xmo Strata and the author of Safety Quality Tricks and Lies: dirty tricks in the British sign industry and 100 questions your sign company doesn't want you to ask, says that the tick-box approach to procurement questionnaires frequently sends a 'subtext' message to bidding companies that they can ignore health and safety related issues, or indeed that they are a secondary consideration after cost savings.

"The customer firm will sometimes have health and safety policies in place, and will make claims about its health and safety credentials. To comply with these policies, bidding companies often have to simply tick a box next to such a statement as 'please confirm your willingness to adhere to the company's health and safety policies' and possibly supply generic documents if requested. The rest of the tender then goes into lengthy details about performance, key performance indicators, price and cost savings, with the final part being 'now name your price', or words to that effect," said Mr Martin. "Any sign firm can win business which is decided on this basis - you hire cheap people, use cheap materials, bullshit where necessary and you go in with a rock-bottom price."

"The weight of wording about price and service often leads suppliers to think that safety is an afterthought and isn't critical to winning the contract. Questionnaires like this are a very loud signal that to the customer, health and safety is a risk which they wish to transfer to the supplier, and I'm afraid that under UK law you cannot do that, however much your procurement department may wish to.

"There is a clearly expressed legal obligation on the customer company to provide a 'safe working environment', and quite often risk transfer in a contract or procurement document simply won't be accepted by a court, which is the ultimate test. As the customer, you have a responsibility to ask contractors to prove that they are competent. Everyone involved in a project should share responsibility for good health and safety practices.

"If someone has an accident and hires a no-win, no-fee lawyer, they will probably sue the party with the deepest pockets, and that is usually the customer. Even individual managers in the customer company may be in peril of criminal proceedings if they are shown to have been cavalier in their attitude towards health and safety."

Mr Martin says that the sign industry is populated by a disproportionately large percentage of senior managers who have no formal management training and who have failed to keep pace with significant advances in health and safety legislation.

"This failure places both them, and their customers, at risk of prosecution and civil proceedings. Customer companies need to be a lot more vigilant to protect themselves from liability claims and reputational and brand damage, arising from accidents resulting in fatalities or serious injury."

Procurement questionnaires need to be a lot more probing about health and safety issues. Any prudent customer will have a documented audit trail showing that it has asked penetrating questions, and done everything reasonable to satisfy itself that the answers it has received are true.

"Modern procurement strategies are often devised by commodity buyers, not service buyers, and this poor technical knowledge base allows unscrupulous contractors a lot of leeway, which poses a risk not just to the contractor, but to the customer company and its managers," said Mr Martin.

The IT environment at ACMC was fundamentally based upon products from Dell and EMC as well as virtual machines from VMware. ACMC is comprised of multiple sites and as a county hospital the organization is under a number of budget constraints. Entisys presented the Citrix XenServer and DataCore SANmelody DR solution based upon XenServer, which provides great savings to the medical center. "Instead of buying another EMC product to replicate the SAN, we showed them what DataCore can do and ACMC was able to complete a replication scenario for pennies on the dollar with XenServer and DataCore," said Thad Morrow, Director of Sales, Entisys. "But it is not so much about the cost savings as much as it is a case where ACMC could use DataCore to support a very distributed environment where multiple vendors' products are deployed and make a full-on disaster recovery solution possible."

"ACMC has struggled to maintain a disaster recovery or business continuity plan, and has had to use what was available to us - an existing data center that is thirteen miles away," said Ed Dullard, Chief Information Officer, Alameda County Medical Center. "ACMC had been a longstanding Citrix customer, so for cost savings it was a natural choice to replace VMware for XenServers."

"We had been a Citrix shop, so we jumped at the opportunity with XenServer," said Dave Bennett, Director of Technical Support, Alameda County Medical Center. "We liked the product and did not see any tangible difference between them and VMware. As for DataCore, we started looking at SANmelody at the request of DataCore partner Entisys. This was the best technology we saw for our needs, whereby we could economically leverage iSCSI for the remote site SAN rather than spend a lot more on a Fibre Channel SAN." ACMC is currently deploying the solution. The initial data pool will start at 8TBs.

For the deployment, ACMC chose Dell hardware to run the SANmelody-XenServer solution. From a software standpoint, the solution cost ACMC at least five times less than a comparable, proprietary hardware SAN. The choice was more than just money, however; it was about flexibility. With DataCore ACMC does not have to worry about matching up machines, nor did they have any issue with the replication software being able to "talk" between disparate hosts. This ease of use and manageability was just as important as upfront cost savings because the IT staff at ACMC is already spread thin, wherein eight IT people support 2,600 users.

The mission of the medical center is to maintain and improve the health of all County residents, regardless of ability to pay. There are six campuses comprising ACMC, including the main hospital and trauma center in Oakland, California, another hospital in San Leandro, California, provides rehabilitation and extended care, a psychiatric hospital in the same location and three ambulatory clinics. In terms of the IT architecture, all of the campuses are linked via a high speed ATM providing 65 MBs of bandwidth.

Two years ago ACMC purchased and deployed a SAN from EMC. "That was the first step we took in order to consolidate storage rather than using individual servers," explained Bennett. "As such it was the first step down this path." Since then, Dave and his team have taken a different look at the way they store data as well as what is stored, particularly in terms of putting a premium on both data and systems recovery. The organization used VMware in conjunction with its EMC SAN. With DataCore, both VMware and XenServers are in place currently - although longer-term the plan is to standardize on XenServer. "The cost difference between VMware and Xen is pretty substantial," Dave added. "As for DataCore, I could see some real advantages in using DataCore in our environment."

Chiefly, Bennett and his team were sold on SANmelody's ability to first and foremost enable them to utilize existing server storage that was not being used, thereby eliminating the need to go out and buy new disk space. Performance was another key selling point. As for price, Bennett comments, "The price was right. SANmelody just made sense when you put the whole thing together - being able to use existing storage where possible and the performance increases we saw firsthand."

The disaster recovery site at the secondary location in Fairmont will enable ACMC to avoid going to tape back-ups. With the DR site, the IT team can pull data right away and pull whole systems up if necessary when the ones at the primary location go down. "Before this, our whole DR plan revolved around just how fast we could get servers and software available to rebuild them," explained Wright Lassiter, CEO, Alameda County Medical Center. "With virtualization technology as it is today, it makes sense to be able to bring systems and data over from another site when the situation dictates that."

ACMC is taking the classic virtualization approach. They have physical servers, which will be virtualized and then replicated on virtual machines. The VMs with those changes will then be replicated to ACMC's Fairmont site and available in the case of any calamity. "DataCore made this project very achievable for a variety of reasons," summarized Morrow. "A real win for DataCore was the simplicity of the technology. Sure SANmelody is affordable, but more than anything DataCore technology is effective. Additionally, the IT staff at Alameda County Medical Center could implement SANmelody quickly, as well own, manage and grow the solution very easily without the need for professional support."

On Monday, October 27, at 12:00 p. m. CST, MeritCare will produce a live, surgical webcast of a Hand-Assisted Laparoscopic Donor Nephrectomy. MeritCare is one of a handful of healthcare institutions in the nation to broadcast live, surgical webcasts and the only hospital in North Dakota to offer both kidney and pancreas transplants. The surgery will be performed by a MeritCare surgical team consisting of Bhargav Mistry, M. D., transplant surgeon, and Timothy Monson, M. D., general and laparoscopic surgeon. Dr. Mistry and Dr. Monson will be available to field questions from viewers from around the world that are submitted via e-mail during the live webcast.

• The surgical procedure: The hand-assisted laparoscopic donor nephrectomy is a less invasive technique for removing a kidney from a living donor for transplantation. The surgery involves the insertion of a laparoscope (surgical telescope) through three to five small incisions in the donorÂ’s abdomen. Using precision instruments that are inserted into the small openings, the surgeon prepares the donorÂ’s kidney for removal while simultaneously observing a magnified image on a TV monitor. When the kidney is ready for removal, the transplant surgeon gently removes the kidney through a small opening. The kidney is then prepared for transplantation into the recipient.

• The benefits: This technique for kidney removal is a vast improvement on the traditional method of open nephrectomy, which requires a longer incision and the removal of a rib. The laparoscopic technique requires several smaller incisions to remove the kidney, allowing minimal disruption to the donorÂ’s abdominal wall. This results in less pain and scarring, a shorter hospital stay and a quicker recovery time for the donor.

MeritCareÂ’s Transplant Program: MeritCare Health System has provided kidney transplants since 1989 with a total of 325 transplants having taken place in the past 14 years. MeritCare Transplant Services currently provides both kidney and pancreas transplants. Transplant surgeon Dr. Bhargav Mistry has performed over 140 transplants since he joined MeritCare in the fall of 2000. The first laparoscopic donor nephrectomy was performed at MeritCare in February of 2002 by Dr. Monson and Dr. Mistry. Twenty-six additional laparoscopic nephrectomies have taken place since that time.

 More than 600 Austin-area musicians – individuals whose livelihoods depend on their sense of hearing - have been provided low-cost medical care and given custom earplugs through a collaboration between Estes Audiology and the non-profit Healthcare Alliance for Austin Musicians (HAAM) called the HEAR Project, which was started in 2009. Each quarter, Estes’ Audiologists and UT doctoral students donate their time for a one-day hearing healthcare clinic to screen, educate and fit HAAM musicians with custom filtered earplugs.

 Since 2005, Estes Audiology has given away more than $30,000 worth of hearing aid devices to Central Texans through the Gift of Hearing program. Each year during the holiday season, Estes Audiology donates two pair of hearing aids to deserving individuals within the community who suffer from hearing loss and lack the financial resources to purchase hearing aids.

 Estes Audiology teamed up in May 2010 with the SERTOMA Club to recognize Better Hearing and Speech Month by asking residents in Central Texas to donate their used hearing aids and turn them in at designated locations. 147 donated hearing aids were collected and then given to the UT-Austin Speech and Hearing Clinic for refurbishing and then distribution to underprivileged patients.

This 5th year anniversary of Estes Audiology coincides with National Protect Your Hearing month. And the facts about hearing loss are unsettling.

 Over 36 Million Americans – more than 4 times the size of New York City – suffer from some level of hearing loss.  50% who suffer from hearing loss are under the age of 65.  A recent report by the Journal of American Medical Association recently reported that 1 out of 5 teenagers are affected by hearing loss, a 33% increase from the 1990’s.

Truly discouraging is that one in three developed hearing loss as a result of exposure to noise, causing noise-induced hearing loss. In other words, Americans are suffering from hearing loss that could have been prevented.

In light of these statistics, Dr. Estes will commemorate this 5-year anniversary of Estes Audiology by encouraging public awareness of the dangers of prolonged exposure to loud noises. “Consumers need to be aware that there is a danger of noise at many sporting events. The level of noise at the 2010 Super Bowl was measured at 102 dB. The level of noise at the World Cup due to crowd noises and the vuvuzelas were measured at a decibel level of 113 which is louder than a chainsaw,” commented Dr. Estes. “It is my hope that our 5-year anniversary will serve as a reminder to avoid loud noises if possible. Tyra Banks Repudiates Permanent Cosmetics on national television. Society of Permanent Cosmetics Professionals (SPCP) responds.

On April 21, 2008, Tyra Banks hosted a live demonstration during what was stated to be a semi-permanent cosmetic procedure using vegetable dyes and minerals. Verika Seddick from the White Tea Spa in New York conducted the demonstration.

The Society of Permanent Cosmetics Professionals (SPCP) would like to clarify statements made during that demonstration. Vegetable dyes are never associated with tattooing. They are dyes used in the food industry to dye vegetables, fruits and meats. The pigments used in the cosmetic tattoo industry are either inorganic or organic (organic pigments are produced through the process of organic chemistry.) Quite possibly Ms. Seddick was mislead by the use of the word "organic." In the world of pigment formulation, organic does not have the same meaning as our society looks upon organic agriculture. Equally disturbing as Ms. Seddick's and Ms. Banks' obvious lack of knowledge of what the demonstration entailed, were their comments regarding tattooing and permanent cosmetics. Ms. Seddick stated that she wanted to make an important distinction, "This is not tattooing and you would never want permanent cosmetics on your face." Tyra echoed her words. A contradiction was soon to follow; Ms. Seddick tattooed a member of the audience's bottom eyeliner on one eye.

Professionals in the permanent cosmetic industry are incensed with this misrepresented and poorly conducted permanent cosmetic (tattoo) demonstration. There is grave concern that thousands and possibly millions of women who potentially could have viewed this demonstration now believe they can have semi-permanent makeup that is not tattooing and that the substance used will be dyes made from vegetables.

Furthermore, professionals are also incensed by the unprofessional and unhealthy manner in which the cosmetic tattooing demonstration was conducted. The following are a few of the known bloodborne pathogens (OSHA) regulations that were not adhered to as well as other matters of concern.
The client was not screened to be a good candidate for an invasive procedure; it appeared as if she were plucked out of the audience without an interview. Ms. Seddick indicated she could do the woman's lips. All professionals in the permanent cosmetic industry know that women of color are often not good candidates for lip color procedures because of their propensity to hyperpigment. Ms. Seddick did not wear appropriate PPE (protective covering) The client's skin was not properly cleaned before an anesthetic was applied. The client was not provided with a protective covering. There was no barrier film on the lamp used for lighting during the procedure. There was no barrier film on the hand piece of the tattoo machine. There was no covering of the cord of the hand piece down to the power supply source. The procedure was conducted poorly in that the eye area was not "gripped" (portions of the eye area held in place for safety and appropriate depth of tattooing.)

The permanent cosmetic industry is due an apology from Ms. Banks. SPCP Position Statement Regarding Semi-Permanent Makeup

It is the position of the Society of Permanent Cosmetic Professionals that pigments (colorants) placed into the dermis using needles are considered to be permanent. Results of fading, color change or lack of color are expected and are the result of factors such as skin variations, and sun fading. Improper application or faulty equipment can also affect the length of time color appears in the skin.

Permanent Cosmetics, Permanent Makeup, and Cosmetic Tattooing are all terms used to correctly identify the process of implanting pigments (inks) into the skin for cosmetic purposes. The term "semi-permanent" is reserved for long-wear topically applied makeup and is misleading to be associated with the tattoo industry. It implies mechanical control of the length of time a pigment may remain in the skin. There are no documented findings to suggest cosmetically tattooed skin can be reverted to its previous unaltered state within a specified timeframe stated by the technician or any other person.

Rationale:
Permanent cosmetics, when performed professionally with reputable pigments/ink, is an act of tattooing and is performed with the clear intention of the resulting implanted color to remain permanently.
Using the name semi-permanent is a crutch for less than favorable results and may also be considered an excuse for not properly explaining the nature of tattooing to a client.

There is no debate that what we do is tattooing. The accepted definition of a tattoo is a permanent mark or design made on the skin by a process of pricking and ingraining an indelible pigment or by raising scars. (Dictionary. com)

It is also common knowledge that many tattoo pigment colors are highly resistant or impossible to remove even with the most advanced laser techniques, so not only permanent, but also some may be non-removable as well. SPCP supplier members who manufacture pigment and devices were contacted to provide information that would support their pigments or devices as being able to provide semi-permanent results. None responded with affirmation that this could occur.
