TransforMED and Cielo MedSolutions Collaborate on Patient-Centered Medical Homes
Cielo Clinic seen as a key element for participants in the TransforMED Medical Home model

Cielo MedSolutions, a provider of healthcare software and web applications, today announced a Preferred Business Affiliate agreement with TransforMED LLC, a wholly owned subsidiary of the American Academy of Family Physicians (AAFP). Through this relationship, Cielo MedSolutions' customers will have access to TransforMED's practice transformation services.
